Policies and Cancellations
CANCELLATIONS AND NO-SHOW  APPOINTMENTS
All treatment appointments scheduled, must be rescheduled or canceled no less than 24 hours prior
to the scheduled appointment time.  Logical Laser will charge a $50 fee per treatment area
scheduled for all appointment changes made within 24 hours of the original scheduled time.  If you
make any appointment within 24 hours, you will not be able to change, cancel or reschedule that
appointment.

LATE FOR YOUR APPOINTMENT?
If you arrive more than 15 minutes late for your appointment, Logical Laser reserves the right to
consider you a no-show.  If the schedule permits, we will perform all or part your treatment areas.
However, this is not a guarantee and Logical Laser cannot be held responsible for late arrivals.  If
you have to reschedule any part of  your original appointment, you will be charged a $50 fee per
rescheduled treatment area. All fees will need to be paid prior to scheduling any future appointments.

ONLINE SCHEDULING
If you submit an online request to schedule an appointment, our Client Consultants will do their best
to get you the appointment time you request. Logical Laser will call you with the date and time of
your appointment.  It is then your responsibility to reschedule or request a change of appointment
that will be more suitable for you.  The online  appointment Logical Laser makes for you is subject to
all cancellation policies. If you are requesting for the following day, you must call to be put on the
schedule.   

REFUNDS
Logical  Laser requires full payment prior to scheduling treatment appointments.  You may cancel
any plan at any time. Logical Laser will deduct the current list price of the used treatment services,
any fees associated with no-show appointments, returned or NSF payments of any kind, plus a
$100 administrative fee.  If a redemption of complimentary issued promotional card from Logical
Laser was applied, the full amount will be deducted prior to disbursement.  The remaining balance
will be refunded or credited back to credit card users.

BOTOX CLIENTS
$100 minimum (10 units) of BOTOX is required.  A valid credit card number must be on record to
schedule BOTOX appointments.  Should you arrive more than 15 minutes late of no-show for an
appointment, the $100 minimum will be billed to the credit card on file is found not to be valid, your
appointment will be canceled by Logical Laser.

GIFT CARDS
Logical Laser sells gift cards for products and services. Only unused cards may  be canceled and
refunded back to  the original purchaser only.  There will be a $50 service fee assessed for
cancellations.  Gift Cards must be present at the time of redemption.  Logical Laser will not replace
lost, damaged or destroyed gift cards.

RETURNED CHECKS
Should a check be returned for insufficient funds, you will be charged a $40 fee.  You will not be able
to make any appointments until the payment amount and fee has been paid and our  records are
current.  Furthermore, Logical Laser will no longer accept this form of payment.  You will either have
to pre-pay with cash, or provide a credit card to secure payments.

CHILDREN
Due to the nature of business at Logical Laser, children under 15 will not be allowed in the laser
treatment room for any reason.  Should you bring a child between the ages of 15-18, they may wait
for you in our reception area.  
No child under the age of 15 will be left unattended in our facility at any
time for any reason
.  If you bring small children for a Laser Hair Removal appointment, you will need
to reschedule, and pay the $50 fee per body area for the rescheduled appointment.

LIFETIME MAINTENANCE WARRANTY
Hair growth and level of hair removal effectiveness will be different for every individual receiving
Laser Hair Removal.  Our maintenance warranty offers a great benefit when you need it. There are
no time limit restrictions.  Should you need continued treatments beyond the 4 in the original
package purchased ( package purchase required for our warranty), treatments may be continued for
the original body area for 50% off of the prevailing single treatment price.

Pre-treatment instructions
Within 12-24 hours before the procedure, shave the area to be treated.  (We can shave and prep
the area for you for $25.  Please notify us when scheduling appointments to add shaving time)
Avoid waxing or plucking the hair and electrolysis 2-4 weeks before treatment.  
Use sunblock with SPF 30 on areas to be treated that are exposed to the sun.
Limit sun exposure, self tanners and tanning beds 2-4 weeks prior to treatment.
Accutane may not be used for 6 months prior to treatment.
Do not use Retin-A or glycolics for 3 days prior to treatment.
Do not use hair removal lotions or hair bleach for 2-4 weeks  prior to treatment.
